Telemedicine services for people with implantable high-energy devices

The Silesian Medical Technology Park Kardio-Med Silesia and GABOS SOFTWARE Sp. z o.o. have established cooperation for the implementation of a project co-financed from the funds within the Regional Operational Programme of the Silesian Voivodeship for 2014-2020 of the European Social Fund for the priority axis: IX. Social inclusion for action: 9.2. Accessible and efficient social and health services for sub-measure: 9.2.6 Development of health services.

Main objective of the project: to increase accessibility to high quality health services for 500 people living in the Silesian Voivodeship (especially from medium-sized cities losing socio-economic functions), at risk of social exclusion due to poor health condition caused by heart failure, by introducing telemedicine services and using them in non-hospital conditions from June 2021 to May 2023.

The project will be implemented in the following types of devices: CRT-D (cardiac resynchronization stimulator with the option of cardioversion and defibrillation) and ICD (cardioverter-defibrillator). The product of the project will be the telemonitoring service, allowing to control the condition of patients with heart failure at home, in effect allowing the deinstitutionalization of forms of support for these people and reduction of factors affecting the social exclusion of patients.

Main tasks in the project:

Inclusion of UP according to inclusion criteria min. 1 month (and more) after implantation of the implantable device by the medical centre/hospital implanting the high-energy implantable device on the basis of medical data (after Echo, ECG, UKG, etc. including assessment of UP according to Barthel scale)
Training of the UP in the use of the telemedicine attachment (attachment provided by the Partner, training during switch-on)
Launch of telemonitoring – Telemedical Centre KMS in cooperation with the inclusion centre and the Partner
responsible for the technological area.
Continuous telemonitoring for 12 months for each UP- 1st line KMS (paramedic/nurse in consultation with doctor). The project leader and partner, based on consultations with doctors and manufacturers of implantable devices available on the market, developed the most effective mode of telemonitoring
Exclusion of patients max. after 12 months – visits/consultation with a cardiologist at the KMS Specialist Outpatient Clinic Team (collection of the device, summary of results, recommendation for further management).
